What networks does CGI contract with?
In order to best meet the needs of our insureds, CGI contracts with provider networks that vary by state.  A complete list of networks can be viewed by clicking here.  CGI does not contract directly with providers.

Am I a contracted provider?
A list of contracted networks can be viewed by clicking here.  If you are a contracted provider with one of these networks, you would be considered a participating provider.  If you have any questions regarding your participation status with these networks, please contact the network directly.

Where do I get a nomination form?
Please use the form attached here.

How can I speed up medical claim payments?
Please confirm you have current claim submission information for your patient by verifying your patient's medical identification card. To further help, please submit a HFCA1500 or UB92 with all required fields completed with appropriate codes.

Where do I send medical claims?
Please refer to your patient's medical identification card for the proper claim submission address.

How do I precertify?
Simply call the Care Coordination number at 1-877-575-4207. Please note, precertification is not required in all states.

How do I verify benefits? 
Please call 1-800-545-8905 in order to verify benefits for a patient. 

What are the dental networks and am I a contracted provider?
CGI contracts with DenteMax. You can see a complete list of providers by clicking here.  If you are not listed, please contact the network directly or complete a nomination form, which is available by clicking here.
